be submitted prior to participation.

1.) Athletics Participation Permission Form
     Now contains user fee payment information
2.) Physical Form
     Proof of physical from certified physician also
     acceptable
3.) Student Emergency and Health Record
     Mailed to all students. Should be returned to high school office over the summer.
